FAQ

How do I file a claim?

Contact the Tallahassee team or your benefit carrier directly to start a claim. Most carriers accept claims online through their member portal, by phone, or by mailing a completed claim form. If you’re not sure which carrier or plan applies to a specific benefit, our team can help point you to the right process.

Typically: a completed claim form for the specific benefit, an itemized bill or Explanation of Benefits (EOB) from your provider, and any supporting documentation the carrier requests (for example, admission/discharge dates for a hospital indemnity claim, or a physician’s statement for a disability claim). Exact requirements vary by carrier and plan — the resource cards above link to the current forms and instructions for each benefit.

If premiums are payroll-deducted, payment changes are typically handled through HR/payroll during open enrollment or a qualifying life event. If you pay by bank draft, use the “Move Coverage to Bank Draft” resource above, or contact the Tallahassee team, to update your banking information directly with the carrier.

This depends on the specific benefit and carrier — some voluntary coverage is portable and can continue after you leave employment (often at a different rate or billing method), while other coverage ends when payroll deduction stops. Contact the Tallahassee team before your transition so we can confirm which of your benefits continue and how to keep them active.

Use the “Schedule a Benefits Review” button on this page, or “Contact the Tallahassee Team” below, to set up a one-on-one conversation. A member of the team will walk through what you currently have and help you understand any gaps or changes worth considering.
